Baker Health Insurance Advisory is an insurance referral and lead-generation service. This policy describes how this website handles information you choose to submit.
What we collect
We collect your name, email, phone number, ZIP code, state, general coverage situation, approximate monthly premium range, requested assistance, and contact preferences. For business requests, we also collect the business name and employee count range.
We record your affirmative consent, its wording and version, the identified referral recipient, and the submission time. We collect campaign parameters, landing-page pathname, referring website information without query parameters, a keyed hash of your IP address for abuse prevention, and limited browser information.
Do not submit diagnoses, medications, surgery details, mental health information, pregnancy information, Social Security numbers, medical records, or detailed health history. We do not request these details.
Optional city and campaign market labels are recorded only when provided in a campaign URL or submitted by you. Geographic information is used for service availability, campaign attribution, and aggregate marketing performance. We do not request GPS location or an exact physical address, append sensitive health information, or infer race, religion, medical conditions, disability, or other sensitive traits from geography.
Why we use it
We use your information to respond to your request, send it to the identified insurance professional, administer follow-up, prevent spam and duplicate records, and understand which sources produce requests. We do not use invasive cross-site tracking.
Who receives your information
Your information may be shared with Christopher Baker, the licensed insurance professional designated to respond to requests through this service. Contact: baker.insurance.advisory@gmail.com. This recipient may contact you by phone, text, or email regarding your request. We do not sell your information to an undisclosed list of recipients.
Service providers that support hosting, database storage, authentication, email delivery, and optional bot verification may process information for these functions. They include Supabase, the configured hosting provider, Resend when enabled, and Cloudflare Turnstile when enabled. Information may also be disclosed when required by applicable law.
Your choices and contact consent
Submission requires an unchecked consent box that you actively select. Consent is not a condition of purchase. You may ask us or the receiving professional to stop contacting you. A contact preference helps guide follow-up, but the consent covers phone, text, and email about your request.
Retention and deletion
We retain requests only as needed for responding, follow-up, and applicable recordkeeping obligations. Administrators can permanently delete a lead and its notes. Retention is reviewed by the operator; this site does not promise a fixed automatic deletion period. Deleted information may remain temporarily in provider backups according to their backup schedules. Information already received by a professional may be subject to that professional’s independent retention obligations.
To request access, correction, deletion, or withdrawal of consent, email tmoran19000@gmail.com. We may verify your identity before acting on your request. Please do not send medical records or sensitive identification documents by email.
Cookies and storage
Admin authentication uses session cookies. A short-lived confirmation cookie stores only your chosen contact method. Session storage may retain campaign attribution for your current browser session; it does not store your form answers. Optional bot verification may use information needed to prevent abuse.
Security
We use server-side validation, restricted administrative access, and database access controls. No system can guarantee absolute security. This website is not designed to collect detailed medical information and does not claim HIPAA compliance.
Adults and US requests
This service is intended for adults making requests about United States coverage. Please do not submit personal information about children; a parent or guardian can provide their own contact information for a family request.
Changes
We may update this policy and will revise the effective date when we do.
